2019 Chapter Ballot (DISTRICT ballot located below)

CHAPTER TREASURER

Candidate: Christina Frank, PT, MBA

Personal Statement: I have a passion for our profession and have enjoyed working at the board level again. Working together, we were able to achieve direct access, which for so long has been a dream. It has also been very exciting to watch the growth of the IPTF and be able to give grants to those researching in PT. It's been amazing to have a front row seat and be a part of a strong team of committed individuals working toward common goals.

Although I love physical therapy, my personal strength is business (MBA) with Treasurer being a role that best fits my skill set. I have been working with budgets related to healthcare for over 20 years. I really enjoy numbers and excel at managing budgets. As an active member of the IPTA for the last 17 years, I’ve held many different positions at the state and district levels. This broad range of experience has prepared me to be a member of the Executive Committee and as your current Treasurer (2018-2019). As with any new role, there is a learning curve and confidence builds with experience. I'd like the opportunity to continue in this role, gaining experience and confidence to best serve our profession and our association.

CHAPTER CHIEF DELEGATE

Candidate: Karen Goodlow, PT, DPT, NCS

Personal Statement: I am honored to be slated to serve the Illinois Delegation in this role. It has been a privilege to serve as the Northern District representative to the APTA House of Delegates for the past 5 years, and in that time, I have learned so much about how our organization functions and have been a part of some very important motions and conversations about the role and the future of the APTA. As Chief Delegate, I feel my strong organizational skills and my ability to see all sides of an issue will help to keep our delegation strong and focus our work. Illinois has a long history of being a strong delegation, and I will help to keep this tradition alive, championing issues and standing up for what we feel is right. I look forward to this opportunity and thank you for putting your trust in me for this important role!

CHAPTER SPEAKER OF THE ASSEMBLY

Candidate: Tina Jonas, PT, DPT, Cert MDT

Personal Statement: I am currently completing the second year as my role as Speaker of the Assembly and feel that I have grown in this position and have much more to offer. In the past year I have learned so much that has helped to prepare me for another productive meeting of the membership and create tools to encourage connections and conversations throughout the year. With the assistance of the Chapter office staff, I have revamped the Assembly webpage and created a new video library to assist members in understanding the Assembly in a new format. I would like the opportunity to continue to contribute toward the Assembly's business and build on these components in the Speaker's role and welcome new ideas to connect with more of the Association's membership. Please come and visit with me at the 2019 Revitalize Conference and Assembly and let me share some ideas on what I'd like to continue to do for our Association. I look forward to our continued conversation. Thank you.

CHAPTER PTA DIRECTOR

Candidate: Jason Magafas, PTA

Personal Statement: I've loved getting involved as much as I can and everyone in the IPTA has been incredibly friendly and welcoming. As a student I jumped into being the SPTA Director and did my best getting my school involvement up. Ever since ending my tenure as a student the professional whirlwind during the first year has finally subsided enough to allow me to resume getting active again and I am excited to be nominated to the ballot and am ready to serve my profession to the highest of my abilities.

CHAPTER DELEGATES - AT - LARGE

Candidate: Samantha Caravette, PT, DPT

Personal Statement: As a student physical therapist and now as a new professional, I have had the privilege of growing with the IPTA and serving in different leadership and advocacy positions. For the 2017-2018 term, I served as the President of the Student Special Interest group, where I led a team of outstanding physical therapy and physical therapy assistant students in advocating for direct access in Illinois as well as promoting diversity in the physical therapy profession.

As a student at UIC, I also participated in many unique volunteer experiences including providing pro bono physical therapy services at the CommunityHealth Clinic in Chicago, participating in a physical therapy service trip to Nicaragua, and organizing/participating in volunteer opportunities with organizations that support athletes with physical and/or intellectual disabilities.

In 2018, graduated from UIC and began my career in acute rehab where I have interacted with many different types of patients and been exposed to a variety of diagnoses. As a new professional, I wanted to continue to be an active member of the Association. As such, I consented to serve as an Eastern District Assembly representative. Shortly after, I was also appointed to the position of Early Career PT Board Liaison. While my career as a physical therapist is still in its infancy, I plan to utilize my current positions as Eastern District rep and Early Career PT Board Liaison to bridge the connection between students and professionals as they transition into their new roles as PT/PTAs. I believe that the continued progression of the IPTA as a leader in physical therapy advocacy relies a lot on the expansion of our membership as well as defining the value of our Association.

As a Delegate-at-Large, I will be a fierce advocate for Illinois and the physical therapy profession as a whole to continue to advance our positions as healthcare providers. I believe my past leadership experience in conjunction with my fresh outlook as a new professional will offer a unique perspective as a Delegate for Illinois. It is an honor to be slated in this position. Thank you for your consideration.

Candidate: Elliot Cohee, PT

Declaration of Candidacy received during Second Call for Nominations

Candidate: Sarah Haag, PT, DPT, MS, WCS

Personal Statement: Over the past 10 years, I've enjoyed becoming more involved with our amazing profession and appreciate all of the people that work hard to move our profession forward. I am able and willing to commit time and effort to be part of the team that represents and works for the physical therapists and physical therapist assistants of Illinois.

Candidate: Sarah Keller, PT, DPT, NCS

Personal Statement: The role of a Delegate in the House of Delegates of the American Physical Therapy Association is one of the most important roles in our profession. Serving as a Delegate for the past four years has been an ongoing honor and privilege for me. I feel strongly that as a membership organization it is vital that the APTA Board of Directors hear and understand the goals and needs of the members, for members to have an active role in the policy decisions of our organization, and that members are involved with the vision of the profession as a whole. I have met many members of the Illinois Chapter and would be honored to represent them at the national level as an At Large Delegate for the House of Delegates. I recognize the work, dedication, and time that is involved in fulfilling this role. Having served multiple years in the IPTA Assembly, on the Board of Directors for the Illinois Chapter, and as an At Large Delegate to the House of Delegates I feel that I have a great level of experience and commitment to the members of this organization and hope to continue to serve Illinois Chapter members as a Delegate in the future.

Candidate: Jennifer Ryan, PT, DPT, MS, CCS

Personal Statement: My role as delegate and chief delegate shaped my professional experience significantly. Being mindful of the role of governance on practice is something that many clinicians are minimally aware of and they benefit from the information and education that I can offer as a result of my experience as a BOD member here in Illinois as well as my national scope experience at the HOD. Seeing the development of a motion from an idea or concern into a motion on the floor of the House of Delegates is always inspiring. Because building a motion requires significant investigation and collaboration with multiple components, you gain a great awareness of the issue from many aspects and it gives you a great opportunity to build a lasting position/charge or policy that will shape practice. My years as a delegate and my experience in building many motions and amendments over the years enriched my clinical role in my acute care setting. Many of the skills that I gained as a member of the House of Delegates and IPTA BOD applied to my inter professional collaborations and program building in my clinical roles.

Candidate: Babette Sanders, PT,DPT, FAPTA

Personal Statement: It is a privilege to have served the APTA and IPTA in many capacities throughout my career. My enthusiasm for serving has not diminished as my career has progressed. I am excited to continue participating as we shape the future of our profession. I have had additional wonderful opportunities to use the leadership skills I have developed through APTA in external agencies. These positions further promote our profession outside of the physical therapy community. As a faculty member at Northwestern for almost 30 years, I hope I have helped to instill the value of professionalism and involvement in many students/alumni. Professional engagement is essential to our continued viability.

Serving as a Delegate to the House of Delegates is one of the most important positions a physical therapist can participate in. I’m honored to be considered for the position. The House is the highest decision making body in APTA. I take the responsibility of serving as a delegate with the strong commitment to help the profession continue to strive to meet the rapidly changing needs of health care, our patients and the profession.

Given my long history of service and continued involvement with the APTA and IPTA, my perspective on the major issues facing the profession and the Association has both the breadth and depth to be an asset to the Illinois delegation and the House Delegates. My current service on the Special Committee on House Documents (SCOHD) has provided an opportunity to examine all of the House documents in detail and through the SCOHD, offer the delegates options to revise our positions/policies to reflect our current vision and priorities. The relationships I have fostered with colleagues from across the country allow me to access information that can help the delegation on motions and candidates. I am eager to continue to help develop the positions, policies and activities of APTA by serving as a delegate. Thank you for considering me in your vote.

Candidate: Sally Taylor, PT, DPT, NCS, OCS

Personal Statement: As a member of the American Physical Therapy Association since 1998, I have enhanced my passion for the field of physical therapy in a variety of ways as a treating clinician, a therapy manager, and an educator. I have had the privilege of practicing in the acute, subacute, and inpatient rehabilitation settings. For the past 14 years, I have served as a therapy manager at the Shirley Ryan AbilityLab formerly the Rehabilitation Institute of Chicago in addition to patient care responsibilities. In 2013, I completed my Doctor of Physical Therapy Degree part-time while working full time and passed the American Physical Therapy Association Neurological Clinical Specialist Examination. Most recently, I was appointed as an Assistant Professor in the Department of Physical Therapy and Human Movement Sciences (DPTHMS) at Northwestern University Feinberg School of Medicine through the partnership established between the Shirley Ryan AbilityLab and DPTHMS.

These experiences will enable me to contribute to the field of physical therapy in the state of Illinois as well as across the country as a Delegate-at-Large. Serving as a Delegate-at-Large supports active participation in the governance of our entire profession. The motions that are reviewed are shaping the field of physical therapy in the present and future state.

My records demonstrate that I am driven and I will continue to demonstrate my personal strengths of being organized, goal-oriented, collaborative, approachable, a strong work ethic and excellent time management skills to contribute to the role of Delegate-at-Large to the APTA House of Delegates. I look forward to contributing to the development of our profession if I am elected to fulfill this role. Thank you for considering my application.

Candidate: Bradford Tracy, PT, DPT, OCS, FAAOMPT

Personal Statement: Greetings. My name is Brad Tracy. I have been a physical therapist since 2003. I am a board certified orthpaedic clinical specialist and a fellow of the American Academy of Orthropeadic Manual Physical Therapists I current practice in the hospital based outpatient environment. I have served as past assembly representative and over the past several years have been actively involved in our association as the Northern District Chair, I have been a past alternative delegate, and serve as the IPTA board-liaison to the chapter’s Telehealth committee.

It is an honor to be slated for this position. Truly, we practice in extraordinary times! Not long ago, our profession embarked on a vision. No long ago, we looked at the year 2020 as a somewhat distance time point. 2020 has now arrived and I look forward to working towards the next horizon, further advancement, and the ongoing progression of our profession. Thank you for your consideration.

Candidate: Patrick Withrow, PT, DPT, GCS

Personal Statement:  I have very much enjoyed my previous leadership positions in the IPTA, which include 3 years on the Ethics Committee, current Eastern District Chair, founding board member of the CESIG, and having first served as an Assembly Rep in 2009. I would welcome the challenge to apply my leadership abilities to the House of Delegates.

In my current role as the DCE at Rosalind Franklin University, I have knowledge of the issues that are important to our profession not only on the local level but nationally as well. I have these conversations with CIs and SCCEs on a regular basis. Serving as a Chapter Delegate at Large would allow me the opportunity to give voice not only to those clinician educators, but to all PTs and PTAs in Illinois.

CHAPTER NOMINATING COMMITTEE MEMBER

Candidate: Sarah Haag, PT, DPT, MS, WCS

Personal Statement: Over the past 10 years, I've enjoyed becoming more involved with our amazing profession and appreciate all of the people that work hard to move our profession forward. I am able and willing to commit time and effort to be part of the team that represents and works for the physical therapists and physical therapist assistants of Illinois.

Candidate: Victoria "Katy" Pippins, PT, DPT

Personal Statement: I graduated from SBU May 2018 and have been working with SSM Health Physical Therapy since Sept 2018. I did my undergraduate degree at UIUC and have been in the Air National Guard for 11 years. I am looking to get more involved in physical therapy associations.

2019 DISTRICT BALLOT

Assembly Apportionment: Note that Assembly PT and PTA Representatives will be determined by September 1st. District allocation of Assembly Representatives is based on the July membership numbers, which APTA provides to the Chapter Office in mid-August. Members who received the most number of votes will be elected to serve as representatives (2020-2021). Per the District Rules of Order, every year districts elect 1/2 of their apportioned Assembly Representatives to serve a 2 year term.
Alternates: All of the remaining names from the ballot, will be listed in vote order, and those members will be eligible to serve as alternate representatives for a one year term only (2020).
Vacancies: Assembly Representatives are elected into position by their district membership. The IPTA bylaws do not provide for representatives to be appointed to fill vacancies. If a district has PT/PTA Representative vacancies after all alternate representatives have been placed, those spots will remain vacant until the next election.
