Fundamentals of Medical Exercise Therapy

Course Description: Therapeutic exercise is now clearly recognized as the cornerstone of interventions for musculoskeletal care. Unfortunately, a method to optimal and precisely dose each exercise is not universally practiced. Well-intentioned clinicians often rely on guesswork to determine the starting and progression parameters for therapeutic exercises. This is problematic in that lack of progress due to under-dosing, injury due to overdosing; or nocebo due to ineffective dosing can result. Further, guesswork does not reflect the skill or expertise of rehabilitation professionals.

It is time for a reset. We must translate evidence-based rehabilitation into daily clinical practice to ensure that all clients receive adequate, intense, and progressive exercise rehabilitation.

This 2-day course presents a step-by-step approach for precision therapeutic exercise dosing. To help therapists build clinical reasoning skills in exercise prescription, therapeutic exercise design, dosing and delivery are taught with an emphasis on the “why” and “how” rather than on specific exercises. As hands-on professions, practical lab components using real clinical scenarios are mixed with interactive discussion-based lectures for skills that can be applied right away in the clinic.

Therapeutic exercise dosing starts with the skill set to perform valid and reliable hand-held dynamometric measurements. Hand-held dynamometry is presented as a comprehensive framework for initial assessment, biomechanical evaluation and the starting point for precision exercise dosing. The skillset is practiced and mastered to be administered efficient and effective in daily clinical practice. This course includes the 240-page hard-cover book: Hand-Held Dynamometry: Guidelines For Daily Clinical Practice.

To achieve optimal outcomes, we urge clinicians to move beyond symptom moderation alone This course focuses on delivering meaningful improvements for clients in 4 outcome domains: symptom moderation; impairment correction; subjective functional performance; and objective functional performance. Further, we teach a systematic structure of inter- and intra-treatment session progression to achieve outcomes at three levels: normalization, optimization and performance.

BACKGROUND: Medical Exercise Therapy is the clinical component of our framework, the MET-odology, which is a framework for clinical best rehabilitation in daily practice, within a value based delivery system to obtain optimal improvement. The concepts taught in our clinical courses are the contemporary development of medical exercise therapy concepts introduced by Oddvar Holten. Interestingly, the concepts of medical exercise therapy are being validated by the latest pain neuro-science research. Belgian manual physical therapist, Leo Wouters, further developed the dosing strategies for clinical practice. With research performed at Andrews University, Berrien Springs MI, USA, we advanced the clinical framework with the latest scientific evidence and incorporated operational, organizational and implementation frameworks which all together we call the MET-odology.

MET clinical courses cover the following topics: Precision therapeutic exercise prescription; training principles; biopsychosocial model; hand-held dynamometry; clinical fatigue test; dosing and dosage; 3 outcome levels; 4 outcome domains; clinical-best-practice; pain science education; treatment session progression; 4 movement patterns; local / semi-global / global exercises; precision design, dose and delivery; value-based rehabilitation.

Course Objectives:

  • Define 7 guiding principles of the MET-odology
  • Identify the 3 contributing factors to clinical best practice.
  • List 3 factors for optimal patient improvement.
  • Describe 3 tactics for creating patient buy-in.
  • Develop 1 treatment plan to achieve meaningful improvements in 4 outcome domains
  • Apply the 4 training principles to therapeutic exercise prescription.
  • Distinguish clinical muscular fatigue from muscle endurance and exhaustion.
  • Operate a hand-held dynamometer (HHD) in 1 biomechanical examination.
  • Execute Clinical Fatigue Tests (CFT) in 2 clinical scenarios.
  • Recognize 3 dosing zones to achieve an appropriate physiological response.
  • Interpret the results of a HHD test for the initial resistance for a CFT.
  • Interpret the results of the CFT to appropriately dose 2 therapeutic exercises.
  • Design 1 precision therapeutic exercise prescription.
