A Randomized Clinical Trial"

Join us for this month's Spotlight on Research Virtual Learning course as Bre Reynolds, PT, DPT, PhD, FAAOMPT presents her research on "Thrust Joint Manipulation to the Cervical Spine in Participants with a Primary Complaint of Temporomandibular Disorder (TMD): A Randomized Clinical Trial."

Temporomandibular disorder (TMD) is a common and costly problem often leading to chronic pain. Moderate evidence exists supporting physical therapy (PT) interventions for individuals with TMD. A known relationship between TMD and the cervical spine exists. Cervical spine thrust joint manipulation (TJM) has been explored in a limited fashion in this population. This lecture will discuss TMD background and the relationship between TMD and the cervical spine. The presenter will describe the prospective longitudinal randomized clinical trial examining multimodal intervention of behavioral education, soft tissue mobilization, and home exercise program for individuals with a primary complaint of TMD. The difference between groups was random allocation to cervical spine thrust joint manipulation or sham manipulation. Fifty participants were followed for 4 weeks and measurements included jaw range of motion (ROM), Numeric Pain Rating Scale (NPRS), TMD Disability Index, Jaw Functional Limitation Scale (JFLS), Tampa Scale of Kinesiophobia (TSK-TMD), Pressure Pain threshold (PPT), Global Rating of Change (GROC), and Patient Acceptable Symptom State (PASS).

Discussion will include the following topics:

  • Background on the diagnosis of Temporomandibular Disorder (TMD)
  • Evidence supporting physical therapy intervention for TMD
  • The relationship between the cervical spine and TMD including previous literature supporting cervical spine intervention in this population
  • Specific to the randomized clinical trial
    • Methods and procedures
    • Sample Description
    • Data Analysis and interpretation
    • Discussion of key take home points and clinical bottom line
    • Limitations
  • Recommendations for Future research in this area
