Vestibular Rehabilitation: Examination and Intervention of Balance and Dizziness Disorders

Course Description: This evidence-based course will focus on providing a strong framework for the physical therapist to effectively evaluate and treat balance and dizziness disorders. Complaints of “dizziness” occur in patients across all practice patterns. It ranks 3rd as the most common reason individuals over the age of 65 seek medical attention. Vestibular disorders can cause disturbing symptoms such as dizziness, vertigo, nausea, anxiety, fatigue, and imbalance which contribute to activity and participation limitations. Many of these individuals can benefit from vestibular rehabilitation. The primary goals of Vestibular Rehabilitation Therapy are to decrease or eliminate dizziness and vertigo and to improve balance, thereby decreasing the risk of falls and improving the quality of life for persons affected. This course will focus on the examination and intervention of patients with vertigo and disequilibrium from vestibular causes, differentiating between peripheral, central, and non-vestibular origins. Emphasis will be placed on various causes of unilateral and bilateral vestibular hypofunction, benign paroxysmal positional vertigo, central vestibular disorders, and multisensory dizziness. Hands on laboratory sessions will enable participants to practice the examination and intervention techniques. Upon completion of this course, the therapist will be able to evaluate a patient with dizziness and/or imbalance and identify if the patient is appropriate for physical therapy intervention or requires additional medical screening. The therapist will also be able to design and implement an effective treatment plan based on current evidence. This course is designed for participants with a basic to intermediate level of experience.

Course Objectives:

Course participants upon completion will be able to:

  • Describe the anatomy and physiology of the vestibular system.
  • Identify the connections between the vestibular, oculomotor and somatosensory systems and how they interact to maintain stability of posture and vision.
  • Recognize and understand the different mechanisms of recovery of the vestibular system and how they impact the course and content of rehabilitation.
  • Describe the pathophysiology, signs and symptoms  of common vestibular disorders.
  • Recognize the clinical significance of diagnostic tests including ENG, caloric testing, rotational testing and posturography.
  • Describe the signs and symptoms of vestibular dysfunction.
  • Complete an interview and examination of a person with vestibular dysfunction.
  • Identify and utilize appropriate standardized assessment tools for use in Vestibular Rehabilitation.
  • Demonstrate evaluation procedures of oculomotor control, balance, and motion sensitivity.
  • Examine and treat Benign Paroxysmal Positional Vertigo (BPPV).
  • Differentiate peripheral vestibular dysfunction from central or non-vestibular causes of dizziness.
  • Develop and implement appropriate intervention plans and measureable goals based on examination findings.
  • Describe factors which affect patient recovery and support the efficacy of vestibular rehabilitation.
