Getting your license

Applying for Your License

This information applies to individuals who have never been licensed in any state, territory or other country before. This information is for those individuals educated in the United States or its territories. 

Qualifications to apply for a license as a PT in Illinois:

  • You must have graduated from an educational program for physical therapists that is approved by the Illinois Department of Financial and Professional Regulation.
  • You must pass an examination approved by the Department to determine your fitness for practice as a physical therapist.
  • You must be 21 years of age and of good moral character.
  • You must apply using the correct forms or apply online and pay any fees to the Illinois Department of Financial and Professional Regulation.

Qualifications to apply for a license as a PTA in Illinois:

  • You must have graduated from an educational program for physical therapist assistants that is approved by the Illinois Department of Financial and Professional Regulation.
  • You must pass an examination approved by the Department to determine your fitness for practice as a physical therapist.
  • You must be 18 years of age and of good moral character.
  • You must apply using the correct forms or apply online and pay any fees to the Illinois Department of Financial and Professional Regulation.


Changing your name on your license

If you have changed your name, for example as a result of marriage or divorce, you must notify the Illinois Department of Financial and Professional Regulation, Division of Professional Regulation.

You are not able to make this change via email or other electronic communication. You must submit a written notice to the Department at the following address:

You must include documentation of your legal name change, such as a marriage license, court order or divorce decree.

New copies of your license with the changed name can be obtained from the Department by paying a fee. There is no fee to change your name, but there is a fee to receive a new license with the name change made. The current fee for a new copy of the license is $20.

APTA and IPTA members please note that changing your name on your license does NOT automatically change your name on your membership. If you want to change your name on your APTA/IPTA membership file, you must call Membership Services at APTA, at 800-999-2782. You do NOT need to notify the IPTA separately, as information is shared between APTA and IPTA on a monthly basis.



Changing your address on your license

Illinois law requires each licensee of the Division of Professional Regulation to keep the agency notified of his/her current address. Changing your address on your license may be done online.

 

If you move and do not change your address on your license, you may not receive your license renewal information in a timely fashion.

APTA and IPTA members please note that changing your address on your license does NOT automatically change your address on your membership. If you want to change your name on your APTA/IPTA membership file, you must call Membership Services at APTA, at 800-999-2782. You do NOT need to notify the IPTA separately, as information is shared between APTA and IPTA on a monthly basis.


Renewing your license

The State of Illinois Department of Finance and Professional Regulation (IDFPR) administers the issuance of licenses for professionals in the State of Illinois.

For Physical Therapists, licenses expire on September 30th of each EVEN year.

For Physical Therapist Assistants, licenses expire on September 30th of each ODD year.

To renew an existing license you must go to the Illinois Department of Finance and Professional Regulation, Division of Professional Regulation website: https://www.idfpr.com/renewals/defaultssl.asp

Upon arriving at the Renewal page you must choose Physical Therapist License Renewal or Physical Therapist Assistant License Renewal by clicking the appropriate profession on the right side bar. (https://www.idfpr.com/renewals/defaultssl.asp).

  • You may choose to renew online by credit card payment.
  • You may choose to renew over the telephone with a credit card payment and PIN number. If you do not know your PIN, you may look on your renewal application to find it. If you do not have a renewal application, you can print it.
  • You may print a renewal application form to locate your PIN or to renew by mail.
