License Changes

Changing your name on your license

If you have changed your name, for example as a result of marriage or divorce, you must notify the Illinois Department of Financial and Professional Regulation, Division of Professional Regulation.

You are not able to make this change via email or other electronic communication. You must submit a written notice to the Department at the following address:

You must include documentation of your legal name change, such as a marriage license, court order or divorce decree.

New copies of your license with the changed name can be obtained from the Department by paying a fee. There is no fee to change your name, but there is a fee to receive a new license with the name change made. The current fee for a new copy of the license is $20.

APTA and IPTA members please note that changing your name on your license does NOT automatically change your name on your membership. If you want to change your name on your APTA/IPTA membership file, you must call Membership Services at APTA, at 800-999-2782. You do not need to notify the IPTA separately, as information is shared between APTA and IPTA on a monthly basis.

Changing your address on your license

Illinois law requires each licensee of the Division of Professional Regulation to keep the agency notified of his/her current address. Changing your address on your license may be done online. If you move and do not change your address on your license, you may not receive your license renewal information in a timely fashion.
